Experience - Mediclaim singed contract copies with completion certificate thereof.
Declaration for cashless facility within 2 Hours.
Declaration for reimbursement claim 15 days
Undertaking that they have not been Blacklisted/Banned/Debarred/Convicted by any Govt./Court of law.
Any other document as require for RFP

Written declaration of Service Delivery conditions and Minimum time prescribed for settlement of Insurance Claims
Agency has to upload signed photocopy/photocopies of agreement/s with the client in support of their experience in providing Medi- Claim facilities along with contract completion certificate/s.
Prescribed time for grant of cashless facility by TPA? 2 Hours, for reimbursement claim 15 days on completion of documentation by the client - Declaration.
Approach or methodology proposed for carrying out the required work - submitted
Bidder must Upload an Undertaking on Organization Letter Head , that they have not been Blacklisted / Banned/ Debarred/ Convicted by any Government / Court of Law.
