Declaration-II as per the format on Govt. stamp paper of Rs. 20 & above-In case the manufacturers/ Principal firm will execute supplies through their Channel Partner/ Authorized Dealer/ Distributor

Declaration-III as per the format on Govt. stamp paper of Rs. 20 & above

GST Registration certificate

Incorpration of the firm as per the factories act - Certificate issued by the Registrar of companies /SSI Certificate to be enclosed

NSIC Registration certificate-If registerd under NSIC

Purchase order / Invoice copies for having supply of 80 percentage of the tendered quantity to chassis-heavy passenger/goods vehicles-manufacturers / any STUs in any one of the last three completed financial years-Refer Annexure -B

Statement showing the value of Plant & Machinery, duly attested by the chartered accountant
