General Particulars

Whether Bidder is Manufacturer/ Authorised Dealer/ Authorised Agent of Manufacturere (Please mention and enclose proof)

Bidder should have experiences in manufacturing and supply of items/ equipments in same name and style for at least 3 (Three) years. (Please enclose scaned copies of purchase orders and at least one performance certificate)

Bidder should enclose detailed statement showing PO Nos. Purchaser's name , Contact Person & Mobile Numbers/ E-mails., Item and Value of supplied items executed during last three year.(Copies of major P.Os may be enclosed)

Bidder shall mention the PAN and TAN number of the firm and to support this, he/she shall enclose scaned copies / submit a photocopy of PAN & TAN certificate.

The bidder shall be registered with the office of competent authority or under statutory bodies at the time of bid opening in the same name and style. In order to support this bidder shall submit.

Bidder should mention the Turnover of last 03 (Three) years. Please enclose scanned copies of Balance sheet & Profit and Loss A/c duly signed by Chartered Accountant and a Certificate issued by CA confirming the Turnover for the last 03 (Three) years.

Bidder should enclose brouchure of the company, showing infrastructure, prodcution capacity, machinery details and technical details.

ISO certified company shall be preferred to others. A copy of valid license/ certificate shall be attached to this effect.

Affidavit regarding any litigation, debarred or blacklisted or history of rescinding work, no near relatives in the concerned division and the informations furnished in the bidding document is correct

The officers of Dairy/Milk Union shall be allowed to inspect the works and/or any document referred to in the tender. If it is found that the declarations made by the tenderer in Technical Bid are false/ misleading/faulty, the tenderer shall be disqualified and the earnest money deposit submitted shall be forfeited.
