6. Undertaking form stating that all the precautions are taken against the labours working in this work and also total responsibility is taken by the Contractor itself.

7. Copy of Self Declaration stating that documents uploaded are true

8. Copy of Annual Turnover certificate for the previous five years from the Charted Accountant (For the Value of A)

9. Work on hands, if no, Nil report has to be submitted (Value of B)

10. Must be eligible for 3AN-B
