Quantities of work executed as Prime Contractor (in the same name) in the last ten financial years in Statement ? III with supporting certificates.

Details of existing commitments i.e., works on hand and works for which tenders are submitted in statement - IV with supporting certificates with documentary evidence such as forwarding slips of agreement and certifiates of value of work done if already executed certain work from the concerned EE along with documentary evidence. refer page (13 clause 3.1(f))

Availability of critical equipment in Statement ? V along with declaration of critical equipment owned/Leased on non-judicial stamp paper of Rs. 100/-. In the prescribed proforma.

Proof of owning QC lab or having tie up with an established quality control laboratory along with equipment required as per NIT

Availability of Key personnel in Statement-VI with their certificates, appointment letters and consent letters

Litigation history in Statement ?VII.

Proof of liquid assets / Valid solvency certificate from any nationalized/ scheduled bank for the required amount in unconditional format as mentioned in the tender document. Special Condition 6: The Bidder shall exhibit proof of ?Batch type Hot Mix Plant? of capacity 100-120 TPH either owned or leased and such Batch Type Hot Mix Plant shall be located within a distance of 100 kms from the last point of working reach, as per G.O. Rt. No. 77, Transport, Roads & Buildings (R.1) Dept. Dt. 27.02.2023 duly self-certified by the bidder.

Special Condition 7: The Contractor shall procure Bitumen & Emulsion conforming to IS, from oil refineries / PSUs such as IOCL, HPCL, HINCOL & BPCL. Executive Engineer should get the authentication of Bitumen / Emulsion invoices from the concerned oil refineries / PSUs before releasing payments.
