Copy of declaration on critical equipment owned or lease on non-judical stamp paper of Rs.100/- with supporting certificate issued by the officer not less than the rank of the Executive Engineer within one year for owned/leased equipment.

In case of leased equipment, supporting leased agreement in Rs 100/- stamp paper with the lessor duly indicating the leased equipment details along with ID proof of lessor and machinery certificate issued by the officer not less than the rank of the Executive Engineer within one year for owned equipment of lessor.
