Eligibility Criteria regarding Class of Contractor Registration: The Class of contractor eligible is given below: Registration with Government of Telangana in appropriate class as per G.O.Ms.No.94, Dt: 01-07-2003, G.O.ms.No.130, Dt: 22-05-2007, GO Ms.No.14, Dt: 31-01- 2015,GO Ms.No.66, Dt: 20-04-2015 of I&CAD Department. The bidder should submit the following particulars invariably in the format specified in the tender schedules with necessary certificates / documents and to be scanned and uploaded online on eprocurement platform, failing which, his tender shall be treated as incomplete and summarily rejected. 2. Copy of PAN card and Latest Income tax returns 2020-21. 4. Copy of GST registration with Government of Telangana/ G.O.I. 5. Certificate on Rs.20/- Stamp Paper notarized regarding no relative is working under jurisdiction of this division office. If no relatives are working, - NIL ? Statement should be furnished, failing which the tenders is liable for rejection. 6. In case the address in various documents (Registration/ ITR/ GST/ etc.,) does not tally, notarized affidavit shall be submitted. 7. The bidder is subject to be black-listed and his EMD shall be forfeited, if he is found to have misled or furnished false information in the forms / statements / certificates submitted in proof of qualification and other qualification criteria. 8. Even while execution of the work, if it is found that the contractor had produced false / fake certificates of experience, he will be blacklisted and the contract will be terminated and the work will be taken over invoking clause 60 (a) of PS of APSS. 9. On Hand Works list. 10. Annual Turnover Certificate for last 10 financial Years for Assessed available Bid Capacity as per formula (3AN-B) should be greater than the the estimate contract value. 11. Any further information can be obtained from the Office of the Executive Engineer, Public Health Division, Karimnagar. SPECIAL CONDITIONS:- Bid capacity is applicable only Estimate costof above Rs. 5.00 lakhs. 12) a) Bid capacity i.e., (3AN-B) will be part of qualification criteria. In terms of para 11 of G.O. MS No. 94, Dt: 01-07-2003. All the works pertaining to this tender call will be evaluated in the descending order of ECV and the TCV of the successful bidder will be incorporated by updating their B value for evaluation of 2nd (and other works down the line) A = Turnover in any one of the preceding ten financial year. (Certificate from C.A. is required) B = Value of commitment of existing works on hand self declaration required. (in the form of proforma) N = (Period of completion in months) b) Payment will be regulated as per the 3rd partly Q.C. reports. For CC roads & drains, etc., running bills will be processed only after getting the results of core tests. c) Contractor has to maintain the following minimum Q.C. equipment at the site at his own cost. i) Cube testing machine (Manual or electrical) ii) Slump cone. iii) Cube moulds Iv) Arrangement for pond curing of cube samples v) Sieves vi) Sand jar 4) the contractor has to get the mix design prepared from any engineering college, by supplying the samples of materials proposed to be used in the work and submit the same for approval before commencing the work. 5) Weigh batching is preferable, but otherwise measurement Boxes shall be mandatorily used by establishing the proportions by value corresponding to the mix design. 6) Photography and video graphy of all important events shall be done at the contractor's cost and the same shall be uploaded every day evening. The documentation shall be done and CD shall be submitted at fortnightly intervals
