PAN Card of the Company / firm / Proprietor.

GST Registration certificate.

IT Return, Balance Sheet and Profit & Loss Account (Bidder / OEM) for last three years i.e. Financial Years 2022-23, 2023-24, 2024-25, if applicable

Purchase Order copies in the bidder's name for having supplied the medical equipment to Government Hospitals / reputed institutions for last three years (2023-24, 2024-25, 2025-26), if applicable

Satisfactory Performance Certificate from the users for the last 3 years from Government Institutes, if applicable

Certificate for at least 5 years warranty. Annexure IX

Certificate for at least 5 years CMC after warranty. Annexure X

Certificate giving the address of the authorized service centre in Hyderabad.

Solvency certificate from bidder

Drug licence of bidder, if applicable

Agreement Between Bidder and Original Equipment Manufacturer (To be submitted on Rs. 100/- non judicial stamp paper and registered)
