Bidders should submit the Data Sheet aligned with product specs, provide PAN and GSTIN copies, canceled cheque, EFT Mandate, and OEM authorization if acting through an Authorized Distributor. The invoice must reflect the consignee GSTIN, and shelf life compliance per DCGI/CDSCO standards is mandatory.

Required documents include: PAN Card, GSTIN, Cancelled Cheque, EFT Mandate certified by bank, Data Sheet proving product specifications match, and Manufacturer Authorization from the OEM for distributors. Invoices must be in the consignee’s name with their GSTIN.

The product must have a minimum two-year shelf life per DCGI and CDSCO; at the date of supply, at least five-sixths of usable shelf life must remain. Ensure documented shelf-life data is included in the Data Sheet.

The option clause allows up to 50% additional quantity beyond bid quantity at contract award, and up to 50% during the contract. Delivery time is adjusted based on the extended quantity using a defined formula with a minimum 30-day extension.

GST reimbursement is based on actuals or applicable rates, whichever is lower, up to the quoted GST percentage. Bidders must manage GST calculations per interim tax rules and provide compliant invoices.

Distributors must provide an Authorization Form/Certificate detailing OEM name, designation, address, email, and phone, proving direct linkage to the manufacturer for product supply to the IAF.
