A bidder quoting for items under import license shall produce all the above mentioned documents except the Performance Certificate. Further the bidder should produce the following documents as formalized as per the prevailing international norms or bilateral agreements between India and the exporting country

Affidavit

Annual Turnover Statement issued on or after 01-04-2026 for proceeding 3 years 2023-24, 2024-25, 2025-26 i.e. from 01.04.2023 to 31.03.2026 issued by the concerned Charted Accountant or Auditor as per Annexure-II

As the bid documents are digitally signed by the bidder while submitting through e-Procurement portal, it is understood that the bidder had signed all the documents including the tender documents within the meaning of digital signature under IT Act 2001

Attested copy of the Balance sheet and Profit & Loss A/c of the Bidder audited by a Chartered Accountant for Last three financial years 2022-23, 2023-24 and 2024-25

Attested copy of the Income Tax returns filed by the Bidder for the last three financial years 2022-23, 2023-24 & 2024-25

Attested PAN CARD copy of the Bidder

Capacity Installation Certificate issued by Drug Controller/chartered accountant, not older than 10 months as per Annexure-III

Details of manufacturing Unit duly signed by Authorized Signatory as per the format enclosed at ANNEXURE -IV. Details of Manufacturing Unit should clearly mention the Manufacturing Capacity of the Unit and the capacity should be adequate to supply the tendered quantity within the stipulated period

Earnest Money Deposit/ Bid security

In case of Primary Manufacturers, Original Manufacturing License along with List of Products permitted issued under the Kits & Cosmetics Act 1940 and Rules, Up to date original Renewed License along with list of products permitted, for the quoted items, is to be uploaded. The bidder must have the valid Manufacturing License for the quoted items, without break, for at least last one year. i.e., from 01.04.2026

Latest GST payment receipt as per Annexure I or in the valid format of the commercial tax department for previous financial year issued by the concerned authorities is to be uploaded along with GST registration certificate, Annual GST returns for last three financial years i.e., 2023-24, 2024-25, 2025-26 and GST returns for the month of March2026, April2026 & May2026

List of Items supplied to various State / National / International Agencies with quantities including Supply to Programs sponsored by W H O / UNICEF during the financial years 2023-24, 2024-25 and 2025-26 i.e. year ending on 2023-24, 2024-25 and 2025-26, if any

List of Kits / Drugs quoted for by the bidder as per Annexure-IV A

Original Market Standing Certificate issued by the Drug Controller / Chartered Accountant for preceding three years 2023-24, 2024-25, 2025-26 i.e. year ending on 31-03-2024, 31-03-2025 and 31-03-2026 as per Annexure-V. The certificate should have been issued on or after 01-04-2026

Original No Conviction Certificate issued on or after 01-04-2026 or not older than 10 months from the date of Tender Notification for preceding 3 years i.e. from 01.04.2023 to 31.03.2026 issued by the concerned Kits Control authorities /Licensing Authorities Annexure-VII

Original Performance Certificate for preceding three financial years i.e. 2023-24, 2024-25, 2025-26 i.e. year ending on 31-03-2024, 31-03-2025 and 31-03-2026 issued by the Competent Authority exercising powers under Kits and Cosmetic Act and Rules, licensing authority as per Annexure-VIII. Issued on or after 01.04.2026
